Dear Peter,

It is about time you come and visit the Netherlands. Many problems 
you mention have already found a solution in my country.
About half of the GP's is connected to local pharmacy networks, thus 
sharing medication history. We receive prescriptions electronically 
from our doctors in my pharmacy. A national format for this 
electronic mail has just been defined by the KNMP, the national Dutch 
Pharmacists organisation to enable exchange between different types 
of software. We as pharmacists, are now slowly attempting also to get 
information from the doctors about prescribing indications.
I know that they are developping a similar system in Sweden now.

As you may know electronic medication surveillance is costomary in 
the Netherlands. Some software packages also have developped an 
Electronic Patient Dossier, in which you can register patients 
contacts and remarks.

I will be present at the First International Symposium on Innovation 
in Pharmaceutical Care, march 21st in Miami Beach. If you are there 
also. I will be glad to provide further information.
