IMHO there is no clear and proper differentiation of this
subject. It is mainly an exercise in semantics.
However, if you choose to pursue the subject, I believe ASHP
(<http://www.ashp.org/>http://www.ashp.org/) has a position statement
on this. A second source is the American College of Clinical
Pharmacy (<http://accp.com/>http://accp.com/).
In either case if you check the archives of this list, (are
they available?) there has been extensive discussion on this topic
in the past.
